Indonesian investigators say deficiencies in pilot training, communication contributed to crash

JAKARTA, Indonesia—Indonesian investigators faulted the design of a flight-control system by Boeing Co., lapses by U.S. regulators, inadequate maintenance and poor piloting skills for the crash of a Lion Air jet that killed all 189 people aboard.
